The Gauteng Department of Education (GDE) has outlined the process for replacing lost or damaged matric certificates and for combining results from multiple examination sittings.
Lost or Damaged Matric Certificate
If your matric certificate is lost or damaged, visit your nearest GDE District Office with:

Your original Identity Document (ID) and a certified copy

An affidavit explaining what happened to your certificate

The damaged original certificate (if applicable)
-
The prescribed fee of R170 (cash at the GDE District Office)
You may also request a statement of results for a fee of R60 while waiting for your replacement certificate.
Processing time:

4–6 weeks if the records are on the system

Longer if records need to be retrieved from the Archives
Replacement Certificate – Change of Status or Combination of Results
If you wrote your matric in more than one examination sitting and need to combine your results into a single qualification (e.g., Senior Certificate or National Senior Certificate), you must apply for a Replacement Certificate: Change of Status.
Bring the following to your nearest GDE District Office:

All your original examination statements

Your original ID and a certified copy

Payment of R170 for the replacement certificate

Optional: R60 for a statement of results while you wait

4–6 weeks for certificate issuance (combination statement is available immediately)
